It took a while, but I found it, on a frontage road in the approximate location given on the back - Route 66, west of I-35.

The space has been empty as long as the Google cars have been driving past, but the satellite view shows the location of the old clock, and the shuffleboard courts, as well as the apron on which the restaurant stood.

I can't imagine that's not it.
