The turning point of her life took place in April 1927, through a meeting with the African-American doctor Eugene Nelson, called by the black maid of Helen after the actress had had a pain in her home in Los Angeles. It was a thunderbolt between the two, followed by the wedding in Tijuana , Mexico, on the following June 28th. In the United States at the time, the episode constituted a serious scandal and was the cause of the marginalization of the actress, who had no more job opportunities and fell victim to drugs and alcoholism.

Not even the separation of the two spouses and the subsequent annulment of their marriage, which took place at the request of the actress on 31 January 1933, resulted in bringing Helen Worthing back into a state of serenity. She was repeatedly admitted to psychiatric hospitals, in September 1935 she attempted suicide and also knew the prison for possession of drugs and drunkenness: she was sentenced to five months in prison in 1939 and one year in 1940.

In April 1944 she was severely beaten by an occasional companion, in 1946 she was found on the street in a confused state. On 25 August 1948 she killed herself poisoning herself with the barbiturates in her apartment full of newspaper clippings and photographs of the golden age of her life.
